Agent loop failures traced to system prompt overrides in production

We traced recurring agent loops to a conflict between user instructions and system prompt safeguards. Adding a priority flag to the context window resolved the deadlock without disabling safety filters. This pattern appears in 15 percent of our production logs during high-load periods.
